It is possible to participate in Lifeline through landline or wireless companies. To enroll in the Lifeline program, contact your telephone company, if you have one, or contact the company you wish to obtain your Lifeline benefit through.
Here are the typical steps to enroll in Lifeline. The phone company may do something different.
You will need to call the phone company and let them know that you believe you are eligible for Lifeline benefits and would like to sign up. You will need to do the following either by phone or by faxing or mailing a form provided by the phone company.
Certify that you are not receiving Lifeline benefits from another company.
Certify that no one else in the household is receiving Lifeline benefits. Federal law limits Lifeline benefits to one account per household.
Provide a copy of documents demonstrating you participate in at least one of the programs used to determine eligibility for Lifeline benefits or that you are eligible based on income. For the programs used to determine eligibility for Lifeline benefits, click here.
